Information We Collect

• Business contact information you provide (name, job title, business email, practice or organization name).
• Practice information you provide about your organization type or specialty.
• Questionnaire draft and submission data provided by clients purchasing the Referral Workflow Review.
• Access and authentication information used to allow you to securely resume and submit your questionnaire.
• Payment status and transaction references. Payment-card details are processed by Stripe and are not stored directly by Markasa Health.
• Standard technical information (such as request logs) needed to operate and secure the website.

Non-PHI Service

The Referral Workflow Review does not request or authorize the submission of Protected Health Information (PHI). You should not submit patient names, dates of birth, patient contact information, medical-record numbers, diagnoses, medical records, insurance member or policy details, authorization numbers, claims, payer responses, screenshots, images, or any other patient-identifying information.

How We Use Information

• To provide the purchased service and prepare your customized workflow review.
• To communicate with you about your order, questionnaire, delivery, and follow-up.
• For security, support, recordkeeping, and to meet legal, tax, accounting, dispute-resolution, and fraud-prevention obligations.

Information Sharing

Markasa Health does not sell client information. Information may be shared only with service providers necessary to operate the service (for example, payment processing and email delivery) and where required by law.

Security

Markasa Health uses reasonable administrative and technical measures designed to protect the information collected through the website. No system is absolutely secure and Markasa Health does not warrant absolute security.

Retention

Business, transaction, and questionnaire records may be retained as needed to deliver the service, support the client relationship, resolve disputes, and comply with tax, accounting, fraud-prevention, and legal requirements. Records required to be retained for those purposes are not guaranteed to be deleted on request.

Your Choices

You may request access to, correction of, or deletion of information you have provided by contacting us at the email below. Some requests may be limited by the retention purposes described above.

Not HIPAA-Regulated

The Referral Workflow Review is an administrative workflow assessment and is not a HIPAA-covered service. Markasa Health does not represent that a Business Associate Agreement is in place for this service and does not claim HIPAA compliance for the public website.
